What is hospital charity care?

Charity care (also called financial assistance) is a program that nonprofit hospitals are required to offer. These programs provide free or discounted care to patients who cannot afford to pay.

Most people do not know these programs exist. Hospitals are not always proactive about telling patients, but the programs are available if you ask.

Who qualifies for charity care?

Eligibility is based on your household income relative to the Federal Poverty Level (FPL). Guidelines vary by hospital, but here are typical thresholds:

• 100% FPL (~$15,060 single / ~$31,200 family of 4): Often 100% write-off
• 200% FPL (~$30,120 single / ~$62,400 family of 4): Often 75-100% discount
• 300% FPL (~$45,180 single / ~$93,600 family of 4): Often 50-75% discount
• 400% FPL (~$60,240 single / ~$124,800 family of 4): Often 25-50% discount

How to apply for charity care

The application process varies by hospital, but generally requires proof of income and sometimes assets. Here is how to get started:

• Call the hospital billing department and ask for their financial assistance application.
• Request the written charity care policy (hospitals must provide this).
• Gather income documentation (pay stubs, tax returns, unemployment benefits).
• Submit the completed application and follow up within 2 weeks.

What if you are denied?

If your application is denied, you have options. Ask for the denial in writing and request an appeal. Sometimes additional documentation or a letter explaining your circumstances can change the outcome.

Even if you do not qualify for full charity care, you may still be able to negotiate a significant discount or payment plan.
