Find the denied service, date, claim or request number, reason code, plan rule, and appeal instructions. Ask the plan for the complete denial and the documents or criteria it used if the notice is unclear.

Appeal routes and deadlines depend on the plan and situation. Use the instructions in the denial and current plan documents. HealthCare.gov describes federal internal-appeal protections for covered plans, but Medicare, Medicaid, employer plans, and state processes can differ.
